Only complete Part II section of these forms and return with ALL forms listed under Standard Licencing in the left column except for the link named Part II
PARENTS
Please click the above links one at a time (left column, unless in Essex then use the middle column, or choose your council from the right column), print off, complete, then return with copy of your child's birth certificate by recorded delivery to the agency. Mark for the attention of BIZZYKIDZ LICENCE DEPT and do not send any other correspondence (ie changes etc) in the same envelope, they need to be sent separately and to their correct dept. Please return all the forms required along with copy of birth certificate and gp health declaration which also appears in the above links. 

If you would like confirmation of receipt, please email licence@bizzykidz.com a few days after posting. 

The agency cannot be held responsible for any items missing in the post, registered post is recommended. Take copy of your gp letter and note the date of signing before sending as it needs to be replaced in 6-mths time, we do not send reminders.
